Purpose of the Intake and Interview Form

The Intake and Interview Form sets the tone for the tax preparation process; after reviewing the Intake and Interview Form with the taxpayer, you should have all of the information you need to complete the return. The IRS requires that all VITA sites utilize the IRS Intake Form 13614-C along with an effective interview for every return prepared at the site.


Why is the I/I Form Imporant?

Verifies Documents

Ensures the client has all of their necessary documents with them

Verifies Eligibility

Ensures the client is within the scope of the VITA program

Identifies Certifcation Level

Identifies the certification level needed to prepare a return

Verifies Certification Level

Ensures volunteers have the certification needed for the returns they prepare 


All VITA Volunteers MUST use Form 13614-C and conduct a thorough interview for EVERY return prepared at the site. This is a requirement by the IRS, and we should take great care to ensure an I/I form is completed for every return. Reviews conducted by the IRS during the 2018 filing season indicated that 100% of returns prepared with a complete Intake/Interview and Quality Review Process were accurate. By comparison only 77% of returns prepared using an incorrect or incomplete Intake/Interview and Quality Review Process were accurate. While these procedures may not seem as difficult as preparing a return, they are an equally important process in ensuring we prepare an accurate return for our clients.
